How to Access Your Patient Portal

If you're 18 years or older, you can sign up now for the benefits of the patient portal.  

When you visit the health department for your appointment, health department staff will start the process of registering.  You must sign a consent form to gain access to your patient portal.

If you haven't signed a consent form and would like to register for the patient portal, please visit our office and staff will register you to gain access.

You can finish setting up your account with a password that will be emailed to you.  Be sure to check your spam/junk folder if it doesn't reach your inbox.
